Nursing Services

Attention 6th grade parents!!!

Dear Stephens Middle School Parent or Guardian,

 

 

State law requires that 6th graders get the shot BEFORE entering 7th grade. Please submit documentation of the Tdap booster to the front office as soon as possible.

 

If your student does not have documentation of the shot, he/she will NOT be permitted to attend class on the first day of school August 29th, 2018.

Estimado padre o tutor de la Escuela Intermedia Stephens,

 

La ley estatal requiere que los estudiantes de 6º grado tengan la vacuna ANTES de entrar al 7º grado. Por favor presente documentación del refuerzo de Tdap a la oficina lo antes posible.

 

Si su estudiante no tiene documentación de la vacuna, NO se le permitirá asistir a clase el primer día de escuela, el 29 de agosto de 2018.

Health Services at Stephens

The health and welfare of your child is very important to the staff at Stephens. Below are some guidelines to help your child have a safe and healthy year.

If your child is injured on or off campus and is seen by a physician the school needs the following information in order to excuse the student from P.E. class, be on campus on crutches or wheelchair and be permitted to use the elevator.

  • The diagnosis  (type of injury)
  • The date the student may return to school
  • Any assistive devises (crutches, wheelchair, etc.)
  • Restrictions (no P.E.; no running, etc.)
  • Length of time to be excused from P.E.

If your child is sent home by the nurse because of a skin rash, conjunctivitis (Pink eye) or other infection, he/she must stay home until symptoms have resolved or a doctor has provided a note stating that the condition is not contagious. The student should bring the note to the nurse's office when he/she returns.

Medications

Students may not bring medications to school. If a student needs to have medication during the school day, you must provide a signed parent authorization as well as a physicians authorization. This medication must be brought to the nurse by an adult and be in an original prescription container.

Students who use asthma inhalers may bring them in their backpacks along with the parent authorization to be on file in the nurse's office.

Illnesses

If your child is visibly ill or running a fever do not send them to school. If you are concerned about your child's attendance please call the nurse and let her know that your child is ill.

Female Students

Sanitary pads are available for $.25 in the nurse's office. Students should carry a pad in their backpack or a quarter to purchase one.
